Keynote panel and roundtable discussion: The case of Kaiser Permanente California
The following cases will be presented as TED talks and then discussed at the round tables, followed by a results presentation.
eHospital Safety Net
eHospital Safety Net is a remote monitoring program that helps ensure the consistency and quality of care provided to patients during hospital stays in Kaiser Permanente Northern California. Nurses at a virtual hub scan patient data generated by the electronic medical record. When they detect a potential care gap, using agreed upon algorithms, they phone a nurse manager at the local hospital to alert them of the patient’s situation. The virtual team also looks for system level issues that don’t require an immediate phone call but are documented for review by the local team at a later date. Health care is complex and this program provides an extra layer of support to nurses and physicians at the bedside. The targeted care gaps were determined by speaking to numerous providers across the organization and asking them a simple question: “What keeps you up at night?” Hospital staff appreciated that the program was implemented in a methodical and judicious manner ensuring that the communication pathways met the needs of the local teams rather than a quality review.
Care Plus Program
The care of complex needs patients is extremely challenging from the care delivery, systems based, and business perspectives. The Care Plus program at Kaiser Permanente Northern California uses predictive modeling to prospectively identify patients that would benefit from a case management team that assumes responsibility for the full spectrum of care a person might require. The design of the program first included input from patients, families, existing teams, and physicians. During the implementation phase of the program, measurements of success included not only clinical/business outcomes but also patient/provider subjective impressions of the experience. Provider feedback is instrumental into developing workflows, validating assumptions, and building upon the early successes of the work.
Acute care in the home
The future of hospital care lies beyond four walls of the hospital. While the hospital setting can provide life-saving treatment, it can also expose patients to risks – infection, delirium, low-quality sleep, lack of ambulation – all of which disrupt the healing process. Kaiser Permanente Northern California has embarked on a journey to build a model that delivers acute care in the home – a setting that maximizes benefits, minimizes risk, and creates an optimal experience for patients. Existing programs within our system have already made progress to this end but also have proved challenging to scale. By utilizing a human-centered design approach, partnering with multi-disciplinary stakeholders, and incorporating learning from existing programs, we aim to produce a model that can be scaled to address a much broader population of patients who currently “have to be admitted” to a hospital building.
